
To our valued patients, welcome to your home for requesting access to your private and secure medical records. To access this information, please visit the new RIMI Patient Portal. To create a new Patient Portal account or for assistance with an existing account, please email Patientportal@rimirad.com. We will need your legal name, date of birth, and a mobile phone number. Alternatively, use the links below to login to the new Patient Portal to view upcoming appointments, download forms, view imaging reports, to make a payment, book certain exams, and so more.
Medical Records (For Patients)
Patients can request a copy of their medical records. This generally involves a fee of $25.00. Per HIPAA requirements, the patient must request their own medical records. For minors, the parent or legal guardian must submit the request. For requests involving power of attorney (POA), please provide this documentation with your request.
- Complete the Medical Records Release Form.
- You may call one of our offices directly to obtain your records. Please call your preferred RIMI Location directly to make this request and coordinate the pickup day and time.
- When picking up your records, please have a copy of your completed Medical Records Release Form.
- Please note that there is a 24 to 48 hours turnaround time needed after placing the request. This may vary for requests submitted on nights, weekends, or holidays.
Medical Records (For Medical Facilities)
Imaging facilities and referring Provider offices can request medical records using the options below. Please ensure you provide detailed information including the required exam(s) and the signed Patient Release Form. We are able to expedite your request with the following options:
- FedEx and US Mail: If you have an account number, provide that information in the request with the delivery location.
- Uploading Images: If you are a facility to which we can upload information electronically, this is how it will be delivered.
- RIMI Provider Portal: If you are a Physician or Healthcare Provider with your own NPI, you can request access to our RIMI Provider Portal by submitting a Provider Portal Application. No staff access can be granted. Accounts are generally activated within 24 to 48 hours after placing the request. This may vary for requests submitted on nights, weekends, or holidays.
